A 68-year-old non-smoker with infection, mobility and a fracture of tooth 22. Atraumatic extraction, a Neodent Z zirconia implant, minor augmentation, then a roll flap and a Cervico-customised healing abutment at second stage. Digital impression and a final crown.

STEP 01
The root of 22 removed by atraumatic extraction. A flap opened for access.

STEP 02
Neodent Z zirconia implant placed, parallelism checked throughout.

STEP 03
Minor bone augmentation to preserve the arch contour; closed by first intention. Insurance constraints ruled out immediate loading.

STEP 04
Second stage. The Cervico Guide tab selects the emergence profile the site needs; a roll flap adds buccal soft-tissue volume.

STEP 05
Implant depth checked with the Cervico caliper: a consistent 4 mm from shoulder to soft tissue.

STEP 06
The implant’s PEEK temporary in the matching well of the Mold, built out and smoothed with composite.

STEP 07
The customised healing abutment secured intraorally. Blue M gel applied to support healing.

STEP 08
The healed site after removal of the custom abutment: the emergence profile is ready for the digital impression and the final crown.
A zirconia implant in the aesthetic zone still needs the soft tissue shaped around it. The roll flap added volume; the Cervico-customised abutment gave that volume its final form, so the crown was delivered into tissue that had already healed to the right shape.
